Default MySQL Database Design - HELP!!! - 05-03-2009, 09:06 AM

I have a little project I'm working on, and would like some help. Here's the situation:

I'm revamping one of my websites in Dreamweaver (HTML and CSS). Visitors to our website can request for free samples to be sent to them. They make this request via a webpage form, then when they hit submit, the information is sent to my email via formmail script.

What I want to do is create some type of database that would keep all this information in one file, so that, at any time, I can use this file as a list of potential clients for mailings or promotions. Of course, visitors will be made aware of this before they sign up. So here's what I want:

1. Clients go to my website and fill out a form. (Fields include NAME, COMPANY NAME, ADDRESS, CITY, STATE/PROVINCE, ZIP, TELEPHONE, E-MAIL ADDRESS)

2. Client hits submit and form info gets saved into a database file, on the hosting server.

3. At the same time, I receive an email containing the form info. If possible, it would be great if this info could already be formatted for Microsoft Word, using predefined fonts, sizes, and position. (Reason being, if it can be formatted, I can just print the email attachment (word file) directly onto my mailing envelopes via laser printer.

4. The database on the server collects all sample requests during the year. At any time, I want to be able to download this file, in Microsoft Word, Excel, or comma separated format, and be able to import the info into Excel or FileMaker.

Can this be done? Is it easy to do? Please let me know...

I'm not sure how this is a MySQL database design question but to answer your final question of "can this be done" the answer is yes. How easy it is depends on your skill level as a developer.
